What is a new entrant safety audit?

A check on a carrier that has just started operating, run once it has been going long enough to have records worth examining - a period the rule says will generally be at least 3 months. The records it asks for have to be made available on request. Our compliance page quotes both of those with the regulation linked.

What fails a new entrant safety audit?

There is a published table of violations that fail it automatically, and the striking part is that nearly all of them are single occurrence - one instance is enough. It reaches drug and alcohol testing, driver qualification, insurance, out-of-service repairs and the driver vehicle inspection report. Two entries behave differently and need 51 per cent or more of the examined records before they count: failing to require a record of duty status, and running a vehicle that has not been periodically inspected.
